If the improved voltage regulator PCB 21 is now necessary for System Blue, where does one obtain the PCB 21 board -- or does one need to build it from scratch?

It is NOT necessary for current systems.
PCB 21 was a 'piggyback' modification added to early  19.1 / 19.2 boards to improve current capabilities, and is not applicable to current systems.
Current production Controller 19.3 or higher does not require the modification.... If you have an unmodified 19.1 or 19.2 board you should refer to Internal Forum http://forum.blitzortung.org/showthread.php?tid=1836

I can see how paragraph 2.3 could lead to confusion, and a very good question, Good catch, MonkeeRench. 
 
2.3 Voltage regulator extension

Unfortunately, the voltage regulators of the boards PCB 19.1 and PCB 19.2 are not adequately adapted to the current consumptions of the boards. This problem is eliminated by the piggyback board PCB 21.
